  4. Standard Contractor Agreement
  5. Standard Employee Agreement

Individuals or other professional corporations or limited liability companies forming a business where individuals are rendering licensed, professional services to the public may form a Professional Service Limited Liability Company.
The Company's principal address can be its physical address or a PO Box and it does not have to be in Arizona.
The owners of the LLC are referred to as the members. In a member-managed LLC, each member has the right to act for the LLC. In a manager-managed LLC, the one or more managers make the decisions and act on behalf of the company. Managers can be members of the LLC, but non-manager members will not have any authority to act for the LLC.
